public health system in England has long been confronted with serious
systemic problems. The problems are apparent with a longer wait time
for medical treatments (even for urgent surgeries) as well as with
an increasing decline of quality. Only primary care is covered. England's health minister recently announced the cancellation of guaranteed maximum wait times for surgeries. Also long waiting periods at general doctors' offices are nothing special anymore. Furthermore, many physicians cannot accept new NHS patients due to capacity overload and have to send them to other doctor's

Health care system and financing of the health insurance: In Germany the health care system is financed by social security tax; in England primarily by tax money. Both countries have a public health care system.
Type of health insurance The public health care in England is assured by the NHS (National Health System). The British NHS was worldwide the first national organization for a free primary care.
Private health insurances: private insurers are available; the benefits are not that good compared to the benefits of private health insurance companies in Germany.
Additional contribution However, not all services of the NHS are for free. Patients have to pay a (subsidized) fee for drugs, eye exams, glasses, and dental treatments unless they qualify for special reasons. On the other hand, hospital stays, outpatient services, and doctor's visits are free of charge. Patients have to pay up to 80% of dental treatments; NHS check ups for women take place every 3 years based on established needs for primary care.
